Fulfillment Rate
Fulfillment rate is the percentage of requested NEMT rides that are successfully completed within a given time period — calculated as (completed rides ÷ total requested rides) × 100. It is the primary operational KPI for medical transportation programs.
Why it matters in NEMT operations
Fulfillment rate is the single most important metric in NEMT operations because it directly measures whether patients are reaching their medical appointments. A high fulfillment rate means the transportation program is reliably connecting patients with care. A declining rate signals driver shortages, scheduling gaps, or coordination failures that need immediate attention.
Industry benchmarks for well-run NEMT programs typically range from 90-95%. Programs operating below 85% often face increased patient complaints, healthcare partner dissatisfaction, and — for Medicaid-funded programs — potential contract performance penalties. Tracking fulfillment rate over time reveals seasonal patterns, staffing adequacy, and the impact of operational changes.
Some organizations calculate two versions: a raw fulfillment rate (all requested rides in the denominator) and an operational fulfillment rate (excluding client-initiated cancellations). Both are valuable — the raw rate reflects overall program reliability, while the operational rate isolates factors within the transportation provider's control.
Example in practice
A behavioral health clinic requests 200 rides in January through their NEMT provider. Of those, 186 are completed, 8 are cancelled by the clinic (patient rescheduled), and 6 are no-shows (driver not available). The raw fulfillment rate is 186/200 = 93%. Removing the 8 clinic-initiated cancellations, the operational fulfillment rate is 186/192 = 96.9%. Both metrics are tracked on the monthly performance report shared with the clinic.
